About Us

College Oak Nursing and Rehabilitation Center is a 120-bed Skilled Nursing Facility and has been in business since 1962. We are a 5-star rated Medicare facility located in the North Area of Sacramento specializing in the care and rehabilitation of seniors recovering from a hospital stay. We are located in the lovely northeast area of Sacramento and boasts close proximity to several of the area’s major hospitals, bus lines, and freeways.

Our goal is to help residents achieve their highest practical level of functioning by combining his or her lifestyle preferences with sound medical treatment. To this end, personalized plans of care are developed by our staff in conjunction with the resident and/or family and with the guidance of the resident’s physician.
